What is the Elmwood Health Center?

The Elmwood Health Center is a free standing, community-based health center that provides a variety of health care services to a diverse patient population that includes people with developmental disabilities, as well as adults and children from the community at large. The Elmwood Health Center is certified under the auspices of both the NYS Department of Health as an Article 28 Ambulatory Care & Diagnostic Treatment Center, as well as the NYS Office of People with Developmental Disabilities as an Article 16 Clinic. Both professional, as well as ancillary services, are available to the patients of the Elmwood Health Center and include Pediatric and Adult Medicine, Gynecology, Podiatry, Physiatry and Psychiatry. Social Work, Physical Therapy, Occupational Therapy, Speech Therapy and Audiometry are also provided. In addition, the Elmwood Health Center has been awarded Level 3 Certification as a Patient Centered Medical Home by the National Commission on Quality Assurance. Level 3 is the highest level possible and is an indication of the quality of the care offered by the providers and staff of the Center.

I like to be prepared. What should I bring on my first visit?

Ideally, you should bring the following information:

  • Photo ID
  • Summary of medical history (include past illnesses, conditions and surgeries)
  • List of current medications
  • Immunization records, especially for children
  • Previous medical records, if possible
  • Name and phone number of pharmacy
  • Medical release of information, signed
  • Legal guardian's name, address, and phone number, legal guardianship papers
  • Health insurance cards and/or ID numbers
  • Heath Care Proxy or we will provide one
  • New Patient Packet (filled out)

What if I get sick after hours?

After office hours, you may contact a nurse at the Elmwood Health Center for advice on an urgent medical need. Call 716.874.4500. If necessary, a doctor will be consulted.

I would like to contact someone in charge of my care. Who is in charge?

Your doctor or nurse practitioner is responsible for your health care. If you have a medical concern or comment, please share it with him or her. If you have a question about an appointment, bill, or your account, call our front desk staff at 716.874.4500.
